Fellow - $150 Must have met all the requirements and been formally admitted into Fellowship of the American College of Surgeons.

2022 Fellow Initiate - Accepted into Fellowship 2022

Associate Fellow - $150 Must have been recognized by the American College of Surgeons as an Associate Fellow.

Retired - $50 Must have been granted retired status by the American College of Surgeons.
Resident - $0 Surgical residents and surgeons in research or surgical fellowship programs who meet the American College of Surgeons requirements for participation.
Medical Students - $0 Medical students in accredited allopathic or osteopathic medical schools, who meet the American College of Surgeons requirements for participation.

Affiliate   - $150 Clinical staff and allied health professionals working in ACS quality programs and departments of surgery, those who assist surgeons and a part of the operating room team, individuals who participate in research and provide surgical education and training, and those who work for a medical association affiliated with the field of surgery.
